Most practices make you call to find out what it costs. Here it is. If none of these fit, we start from the closest one and tailor it to your situation.

Steady Check-In

Tier 1

$325 / month
  • 1 home visit a month
  • Phone check-in with your parent every 2 weeks
  • Coordination that keeps the picture current and steps in as needed
  • 2 hospital events a quarter

Guided Care

Tier 2

$850 / month
  • 2 home visits a month
  • Weekly phone check-in with your parent
  • Active coordination between visits, centered on the primary care doctor, with the medication process run end to end
  • Up to 1 appointment attended in person a month
  • Monthly call with you
  • 3 hospital events a quarter

Full Circle Care

Tier 3

$1,550 / month
  • Weekly home visits
  • Weekly phone check-in with your parent
  • Full-team coordination: I'm the hub across every provider and the pharmacy, driving the plan
  • Up to 2 appointments attended a month
  • Priority same-day response to routine questions, not just emergencies
  • Monthly call with you
  • 4 hospital events a quarter

What never changes

Some things don't scale with what you pay. These hold at Tier 1 exactly as they hold at Tier 3.

A nurse's clinical visit, every timeVital signs, a focused nursing assessment, observations, and acting on what I find. Not a check-in, at any level.
Appointment schedulingBooked, confirmed, and kept on track, whatever your plan.
A visit report after every visitWhat I saw, what changed, medication status, and what I'd recommend.
Education for you, your family, and any caregiverI explain the conditions in play, what they mean day to day, the warning signs, and how to help.
Same-day contact on anything urgentA fall, a medication error, signs of illness, a safety hazard, you hear from me that day. Routine questions get a reply within one business day.
A visit within 2 business days of any ER dischargeEvery plan. This is when things go wrong, so it isn't something you have to pay extra for.
Medication reconciliation after every hospital stayLine by line against the pre-hospital list. Timing varies by plan. Whether you get it does not.
Hospital visits are extra visitsAnything triggered by an ER trip or admission is on top of your monthly visits, never counted against them.

You can move up a plan at any time. To move down a plan or cancel, I ask for 30 days' notice. Private pay only: I'm not enrolled with Medicare, Medicaid, or private insurance, though some long-term care policies include a care coordination benefit worth checking.
