A directive only works if someone at the bedside knows it exists and speaks up. I make sure the medical reality follows what your client put on paper, and I tell the family when it doesn't.
A healthcare agent who can actually act
The agent your client named needs current clinical information to make the decisions you built the document for. I keep them informed, in plain language, before the crisis and during it.
End-of-life navigation
When it's time for goals-of-care conversations or hospice, I help the family understand the options and make sure the care your client receives matches the wishes they wrote down with you.
A current picture before you draft
When you want a real-world read on how a client is managing at home before a signing or a plan update, my written in-home assessment gives you one, in your file, with a date on it.
A referral that reflects well on you
I treat the people you send me the way you would want them treated, and I am honest about what I can and cannot do. If I am not the right fit, I will say so. Your name is safe on the referral.

You focus on the legal work. I take on the day-to-day of the client's care, so the family has a professional in their corner and you know they are in good hands.
Someone actually in the home
I make scheduled home visits, assess what is really going on, coordinate the doctors, and follow through, so the plan does not just live on paper.

Your client sees me, the same registered nurse, at every visit and on every call. Nothing gets lost in a handoff.
